UnitedHealthcare

In 2014, UnitedHealthcare and Arizona Care Network launched a commercial product to improve care quality and reduce costs for thousands of Phoenix-area residents enrolled in UnitedHealthcare’s employer-sponsored health plans. 

Following the success of their initial product, UnitedHealthcare partnered with Arizona Care Network on UnitedHealthcare Navigate.

Cigna Collaborative Accountable Care

Cigna-Collaborative Accountable Care (CAC) is a commercial product offered to Phoenix-area residents enrolled in Cigna employer-sponsored health plans. Arizona Care Network is a preferred provider network for Cigna-CAC in Maricopa County.

Arizona Care Network is also the designated provider for Tier 1 benefits in Cigna's health plan for Abrazo employees in the Phoenix area. 

Direct to Employer (D2E)

Connected Care with ACN

Connected Care is a direct-to-employer health insurance product available exclusively to Intel employees. The plan offers a comprehensive, patient-centered approach to health care services within one network. Arizona Care Network is the exclusive provider network for Connected Care. UMR Dignity Health

Arizona Care Network is the Tier 1 provider network for Dignity Health employees in Arizona. 

Medicare Accountable Care Organization Plans

Medicare Shared Savings Program - MSSP

The Medicare Shared Savings Program (MSSP) was designed to encourage coordination among providers to improve the quality of care for Medicare fee-for-service (FFS) beneficiaries and reduce unnecessary costs. Like other ACO models, the Medicare Shared Savings Program rewards ACOs that lower health care costs while meeting performance standards on the quality of care.

Next Generation ACO

Built upon the success and experience from the Medicare Shared Savings Program, the Next Generation ACO Model is a new opportunity in accountable care. The model sets predictable financial targets, enables providers and beneficiaries greater opportunities to coordinate care, and aims to attain the highest quality of care. It will allow these provider groups to assume higher levels of financial risk and reward than are available under the current Pioneer Model and Shared Savings Program (MSSP). The goal of the Model is to test whether strong financial incentives for ACOs, coupled with tools to support better patient engagement and care management, can improve health outcomes and lower expenditures for Original Medicare fee-for-service (FFS) beneficiaries.

Bright Health with Arizona Care Network

Arizona Care Network is the exclusive provider network for Bright Health’s Medicare Advantage Plan in Maricopa County. Medicare Advantage is an alternative to traditional Medicare that's offered by private insurance companies. By partnering with Arizona Care Network, Bright Health's Medicare Advantage plan offers patients a cost-effective path to high-quality treatment.

Cigna-HealthSpring

Arizona Care Network is a preferred provider network for Cigna-HealthSpring, Cigna's Medicare Advantage plan in Maricopa County. Medicare Advantage is an alternative to traditional Medicare that's offered by private insurance companies. By partnering with Arizona Care Network, plan members can access a cost-effect path to high-quality treatment.

Medicaid

Mercy Care Plan

Mercy Care Plan is a not-for-profit health plan serving Arizona Health Care Cost Containment System (AHCCCS) members. This plan, in particular, uses ACN's Care Coordination team to serve as the member's liaison between providers for truly integrated care. Care Coordinators and Behavioral Health Coaches help remove roadblocks to care and find resources to help improve the member's health. This leads to improved outcomes and reduces the total cost of care.
