Welcome to Arizona Care Network

Arizona Care Network (ACN) is a physician-led, physician governed clinically integrated network comprised of like-minded, high-performing physicians who share a passion for improving quality of care and creating a better patient experience.

As an Accountable Care Organization (ACO) participating doctors, hospitals, and other health care providers come together to provide coordinated, high quality care to our patients. Coordinated care helps ensure that our patients, especially the chronically ill, get the right care at the right time, with the goal of avoiding unnecessary duplication of services. This level of coordination is highly efficient and reduces costs.

ACN’s board leadership and governing committee members are dedicated to making informed, clinically-sound decisions on behalf of the organization which align with the best interest of network physicians and their patients. We have developed a collaborative, evidence-based care delivery model that allows physicians to focus on what they do best—improving the health of the patients they serve.

ACN is uniquely structured to allow our physicians to maintain their independence while giving them the opportunity to leverage shared resources, technology and support that will prepare them for the future of healthcare delivery.

Arizona Care Network is a partnership between Dignity Health and Tenet Health Systems, (Abrazo Community Health Network). ACN also includes Phoenix Children’s Hospital, the most comprehensive pediatric care in the state and one of the largest children’s hospitals in the country.

ACN’s physician community is composed of more than 1,100 Primary Care Providers and 3,900 Specialist Physicians, including nearly 300 general pediatricians and 80% of local pediatric specialists. Additionally, more than 1,250 locations make up the continuum of care for patients, including:

  • More than 35 Skilled Nursing and Home Health Agencies
  • 86 Imaging Centers
  • 24 Retail Clinics
  • 44 Urgent Care/Emergency Centers
  • 2 Children’s Hospitals
  • 14 Acute Care/Specialty Hospitals
