Care Coordination

Arizona Care Network’s Care Coordination team offers a full range of healthcare services, customized to your personal needs.

 

ACN’s Care Coordination team gives you individual support from Registered Nurses, Navigators, Social Workers, and Behavioral Health Coaches who provide trustworthy health information and help you follow your doctor’s plan of care to achieve your best health.

Physicians manage about 20% of a patient’s health; the rest is determined by other things going on in your life. Our team becomes part of your healthcare team by working with your doctor to improve your health by connecting you to services and resources you need to get healthy and stay healthy.

A Care Coordination team can:

  • Meet with you over the phone or in person
  • Get the answers and resources you need
  • Help you manage multiple medications so you don’t have negative interactions
  • Coordinate healthcare appointments and even attend appointments with you if you wish
  • Teach you tips for self-care

There is never a bill for Care Coordination services. ACN provides this support at no charge to the patient because keeping you healthy is important and reduces healthcare costs associated with worsening health and hospitalization.


How Do I Connect to Care Coordination Services?

  1. If your doctor believes you would benefit from additional information, services, or support for your care, they may refer you to Care Coordination.
  2. If your health history or current status suggest you may need additional help managing your health, one of ACN’s care coordination team members may reach out to you.
  3. You can also refer yourself. We will confirm if you are eligible.

Examples of How We Can Help You

  • You have a new or chronic illness and want to learn how to manage it better or need help following your doctor’s treatment plan.
  • Your medical tests or assessments show that you may be at risk for worsening health and you want to stay healthy and out of the hospital.
  • You are facing transitions of care. If you do have to be in the hospital, the transition to a skilled nursing facility, rehab center, or even home is a time when each decision is important to protect your health and keep you out of the hospital.
  • Your doctor recommends care coordination. Some doctors have Care Coordinators embedded in their practices to make this support even more convenient for patients.
  • We’re healthcare insiders with the connections and expertise to help guide you through the complex healthcare maze. We can help you with many situations and customize the best team to meet your needs. We communicate with your doctor every step of the way.
